| Description |
This paper summarized the practice of integrated healthcare model in hospice care of elderly patient with end-stage lung cancer. A multidisciplinary team was established, and patient-centered interventions based on integrated healthcare model were carried out to improve the psychical, mental, social and spiritual health of the patient. By meeting the diversified needs of the patient and his family members, the hospice care based on the integrated healthcare model helps to relieve the patient's physical and psychological pain, and ease grief process of family members. (本文总结整合照护模式在1例肺癌终末期老年患者安宁疗护中的实践经验。组建多学科协作整合照护团队, 以患者为中心, 为患者及家属提供身体、心理、社会、精神全方位的照护, 满足患者和家属的多样性需求, 有效减轻患者生理和心理上的痛苦, 缓解家属的悲伤情绪。)
